Cost: $100 per month

This membership is intended for current patients who are looking to take a proactive approach to their health. New patients are welcome to join after completing a new patient office visit. The cost of your new patient visit will count as your first month's membership payment, consecutive payments will then reduce to the $100/month. New patients who join our Proactive Health Community in Hudson WI will receive a voucher for their first month's 30 minute Recovery Session because this service is not inlcuded in a new patient appointment.
